How to schedule an appointment
Call or email me to request an appointment stating which type of consultation you would like. If you are obtaining the Initial Core Service you will be directed to a questionnaire to provide the information needed for this service.

If you’re ready to take the first step toward securing the Social Security disability benefits you need, please fill out a confidential client questionnaire. This form will take approximately 10 minutes to complete and will help me understand your unique situation, including your medical history, work status, and how your disability affects your ability to work.
Your responses will enable me to identify key strengths and weaknesses in your case and prepare for a thorough and productive consultation. If completing the questionnaire is difficult, simply answer as much as you can, and we’ll discuss the rest during our session. Together, we’ll chart a path forward to enable you to navigate the process with clarity and confidence.
